FHM Certificate Holder

FHM has prepared a graduation souvenir, a certificate holder, for you to collect after completing the graduation survey. Here are the steps to get your certificate holder:

Step 1: Complete the Online Graduation Survey.

Step 2: Visit the FHM Faculty Office (B2-G/F-04) on 25 November 2023 (Sat) from 9:30 to 10:00 am and 11:00 am to 12:00 nn.

Step 3: Present your (1) HKID/Passport/Student ID, and (2) screen capture of the “thank you message” after completing the survey to the staff.

Step 4: Collect your graduation souvenir!

If you are not able to collect it in person, you can authorise a representative to collect for you with:

(1) the completed authorisation letter with signature; and

(2) the supporting document of the identity of your representative (e.g. HKID card/Passport/Student ID, digital copies are also acceptable).

You can also collect the certificate holder at the FHM Faculty Office (B2-G/F-04) from 27 November 2023 (Monday) to 15 December 2023 (Friday) [Mon-Fri 9:00am-12:00nn and 2:00pm-5:00pm, except public holidays] and follow the steps above. If you have any queries, please contact:
